I have some decorative trim (MDF 1x4s) above my fireplace. The studs are on the outsides of the outlet boxes. What would be the best way to secure the mount? Install some additional 1x4s on top of the stud locations to bring it flush with the rest of the trim?

I would put some nice looking 1x4's or 1x3's on the outside of the boxes into the studs like you said. Paint to match and mount into those furring strips. They don't have to go all the way to top and bottom if that will be seen on top and bottom of the tv. Just gives you a flat secure mounting location.

The mount will probably sit flush on the 1x4s

Mount it as normal. It will sit flush regardless.

I agree to mount as normal. it will sit flush on the 1x4. hitting the studs with your lag screw is obviously the most important part. you will have plenty of screw length to reach deep enough into the studs to hold todays TVs.
only question I have (not that it really matters) is how wide is your mount? will it sit flush on a single 1 x 4 or catch 2 or 3 of them? if you can catch 3 thats best, if 2, you could still slide the TV left and right to center on most mounts. I think adding 1x4s is just extra work.

The mount is just over 16”. Just wide enough to screw into two studs 16” OC. There are no studs behind the decorative MDF pieces. The two outlet boxes are nailed into the studs I can reach with the mount.

If your mount is only 16" would definitely do the 1x4 underneath. It might be fine without but if it were me I'd want my mount sitting flush at all 3 points (left, middle, right). Might bend the mount when tightening the lag screws if only point of contact is in the middle but idk

Bending the mount would be my concern. I would match the trim pieces and run 2 horizontally between 2 vertical boards. Lag through them into the studs.

Agreed, I was the mounting spots flush with the center board. Flat and stable.

Would absolutely put mdf behind to fill the gap.

I'd just grab or make some 3/4" spacers if you bolts are long enough. No need to screw in a spacer into the wall, a hole saw and drill can make quick spacers out of some 1X lumber. Your mounting bracket might even come with the correct spacers, some mounts come with WAY too much hardware and some don't have enough.
