A warrant is issued based upon an affidavit of probable cause. The affidavit lays out the evidence and the cause for the search. The actual warrant is just an order.

The warrant is just a standard piece of paper stating they have permission to search and who it's signed by. It DOESN'T state why they have asked for or need the warrant. That is in the Affidavit.
