The device in your pictures is an ONT. Maybe it's old and abandoned, but that's what it is.
Enable "Manually specify public port " and specify port 32400. Turning that option off means PLEX will try to configure your router automatically using uPnP. Your router probably doesn't support uPnP.
If that doesn't work, temporarily turn off the Windows firewall to see if that's the problem.

this is correct. At&t has removed all versions of uPnP from their hardware. There is zero allowance for uPnP on at&t fiber internet services using their hardware.
