St Thomas is USVi and has their own entry requirements you will have to meet. BVIs has separate requirements that are more strict - and they enforce them. This usually requires a negative test before arrival and another test on arrival. As long as you stay within the BVI islands, you only need to do the entry test once. Will need to do yet another test before returning to USVI and USA - best to bring a home kit and do this on the boat as testing sites in BVIs are not very convenient. The BVI entry rules may well change before your trip, so best to monitor their official government website.

And there would be multiple entry fees required if one were to leave for some unknown reason. You come to the BVI you check in. You leave you check out. Money changes hands every time you do this.
