Hydroxychloroquine: NHS staff to take drug as part of global trial

A major trial to see whether the malaria drug hydroxychloroquine, taken by Donald Trump to protect himself against Covid-19, can prevent health workers from getting the disease is beginning in the UK and other countries.

Trump’s revelation that he was taking the drug provoked an outcry from doctors, who warned it could have dangerous side-effects on the heart. The two forms of the drug, chloroquine and hydroxychloroquine, can cause abnormal heart rhythms, which could be of additional concern in somebody who is overweight.

But preventive measures against infection are urgently needed for health and care workers on the frontline and with no vaccine expected anytime soon, a major trial has begun, aiming to test the protective effect – if any – in 40,000 health workers. Brighton and Sussex University hospitals and the John Radcliffe hospital in Oxford have begun enrolling volunteers.

The study is being led by the University of Oxford and the Wellcome Trust, and supported by Mahidol Oxford Tropical Medicine Research Unit in Bangkok, Thailand, where the drugs have been commonly used in the past to treat malaria. Participants will be recruited from Europe, Africa, Asia and South America.

“Covid-19 is a major risk for frontline healthcare workers around the world,” said Prof Sir Nicholas White of the University of Oxford, who is based in Bangkok and is joint leader of the trial, called Copcov.

“We really do not know if chloroquine or hydroxychloroquine are beneficial or harmful against Covid-19. The best way to find out if they are effective in preventing Covid-19 is in a randomised clinical trial. That’s what Copcov is – and why we’re doing this study.”

His co-investigator, Prof Martin Llewelyn at Brighton and Sussex medical school, added: “Even though the lockdown has brought the rate of infection right down in the UK, healthcare workers will continue to be at risk of Covid-19, especially as measures are relaxed. A widely available, safe and effective vaccine may be a long way off. If drugs as well tolerated as chloroquine and hydroxychloroquine could reduce the chances of catching Covid-19 this would be incredibly valuable.”

Hospitals are setting up to join the trial in Thailand and south-east Asia, Italy, Portugal, Africa and South America. However, the welcome drop in the number of cases in many countries will make all ongoing trials in drugs and vaccines a slower process, as fewer health workers are infected or protected.

Health workers who take part will be randomly allocated either the trial drug or a placebo. In Asia, the trial drug will be chloroquine and in Europe and Africa it will be hydroxychloroquine.

Chloroquine and hydroxychloroquine have been used for treating malaria for more than 60 years and are generally regarded as very safe drugs. Until recently 300m chloroquine treatments were given for malaria every year – it is less used now because of resistance and the development of artemisinin drugs.

Side-effects are usually mild and include stomach upset and nausea, headache and temporary blurring of vision, but too high a dose can be dangerous and lead to death. There is concern that the drugs can prolong the electrocardiogram QT interval, causing an irregular heart rhythm. People who already take other drugs that could cause this side-effect should be careful, say the scientists on their trial website, “but for healthy people who are not taking regular medications which prolong the QT interval, there should be no concerns about cardiotoxicity”.

Gov. Gretchen Whitmer’s Department of Licensing and Regulatory Affairs literally threatened all doctors and pharmacists in the state who prescribe or dispense hydroxychloroquine to treat COVID-19. The agency’s March 24 letter warns physicians and pharmacists of professional consequences for the prescribing of hydroxychloroquine (and chloroquine). Beyond the rational recommendation against hoarding as production of this medication needs to be ramped up, the letter deviates into open threats of “administrative action” against the licenses of doctors that prescribe hydroxychloroquine.The letter also instructs pharmacists to ignore physician orders for this medication. Due to the debate over a pharmacist’s right to refuse to fill medications that go against their religious beliefs, this could place pharmacists in the unprecedented position of being told that they must fill prescriptions that violate their “ conscience (religious belief)” but must not fill prescriptions to treat COVID-19.

Even worse, the letter indicates health care providers are “required to report” their fellow physicians who are prescribing these medications. This draconian measure carries ominous Gestapo-like overtones of neighbor reporting neighbor to “authorities.”

During a time of crisis, in which physicians continue to see patients despite not having enough protective gear, this threatening, authoritarian stance from our governor is counterproductive at best.What makes this directive more of a head-scratcher is that the same day the state issued its threatening nastygram to Michigan’s health care providers, Whitmer’s counterpart in New York started clinical trials of the very same drugs.

With his state now the nation’s pandemic epicenter, and with the blessing and help of the president and FDA, New York Gov. Andrew Cuomo brought in 70,000 doses of hydroxychloroquine, 10,000 doses of Zithromax and 750,000 doses of chloroquine.

The US military has joined an investigation into a plane that crashed in eastern Afghanistan on Monday.

Local officials initially said the aircraft that came down in Deh Yak district, Ghazni province, belonged to state-owned airline Ariana.

But the airline denied this, prompting questions about the origin of the aircraft and the cause of the crash.

Video and images posted on Fars news agency's social media appeared to show a Bombardier E-11A jet, a US Air Force plane.

The US military uses this type of plane for electronic surveillance over Afghanistan.

US Army Maj Beth Riordan, a spokeswoman for US Central Command, told Associated Press news agency that it remained unclear whose aircraft was involved in the crash.

Another US defence official told Military Times: "We are aware of the reports and are investigating. At this point, we cannot confirm it is a DOD [Department of Defence] asset."

After initial speculation that it was an Ariana plane, the airline issued a strong denial, saying it only had two planes in the air and that they were safe.

Mirwais Mirzekwal, chief executive of Ariana, told Reuters: "It does not belong to Ariana because the two flights managed by Ariana today from Herat to Kabul and Herat to Delhi are safe."

Ghazni provincial governor Wahidullah Kaleemzai later told private broadcaster TOLOnews: "There is no exact information on casualties and name of the airline."

Afghanistan's aviation body also said that no civilian planes had crashed, and Taliban spokesman Zabihullah Mujahid told the BBC that his group had not yet located the aircraft.

Ghazni police commander Ahmed Khalid Wardak told the BBC that there was no information about casualties, and that it was unclear what had caused the plane to crash. He added that the aircraft had been set on fire.
