PDF Form for Service Techs

I have a crew of technicians who preform field jobs that require getting a ticket signed that spells out what they did/for whom, what parts were used yada yada

I turned this PDF into a form and i would like to give all the techs an ipad so they can digitally fill this out and ask the customer to sign the ipad. I would like this emailed to review/edit. Can anyone recommend an app that i can upload my form into and can use?

Needs to be user friendly for service techs

I've used Right Signature in the past. Just overlay the fields on the pdf. User friendly and you can reuse the same link.

Adobe can do this with ease. You may need to pay for it, though

We do exactly that but handed out Surfaces instead. The techs use Foxit reader to fill out the form and collect the signatures. We went to this about 2 years ago and it is working well.
