Really new to this, I just got the 12 month online ps4 account for $59.99, do I also need to purchase Netflix or is it included?

Netflix is separate unless it was bundled in the deal.
If you already have an account just put that in the credentials on the screen.

for future reference, you don't need PSN (playstation online) to watch netflix. PSN is for gaming online.
