I have watched footie for my entire life and have never seen this.

I think it has something to do with live ball vs dead ball. The ref can't blow a play dead just to show a yellow card and give the ball back to the same team.

Canadian press says they had the game taken away by the 6 second call. Had the ref been warning the goalie before about speeding up or the call would be made?

The goalie was quoted as saying that the ref had warned her in the first half.

I knew as soon as the whistle blew, as the only time I've ever seen it called (at any level) was against my team in a co-ed tournament in Austin six or seven years ago.

Obviously, the USA brought this to the refs' attention prior to the match, because they were clearly watching and had warned Canada's keeper in the first half.
