I would say the difference is W just quietly let them treat him that way because it was “beneath the office” to respond. And for that, he was still called Hitler and a racist. Trump responds in kind and that pisses them off even more.

He crashed the party after the Democrats truly believed that another Republican would never win after the "scandal free" presidency of Obama. The vitriol and hatred were there for Romney/Ryan and McCain/Palin presidential tickets but it quickly dissipated since they lost.
