Yes. Scanned copy of CCP. Along with copies of birth certificate or passport to show proof of US citizenship. Copy of driver's license for proof of age. Copy of proof of membership in a CMP affiliated club or program. And don't forget to complete the 4 pages of the order form, of which page 2 needs to be notarized.

1st page is the customer ID, liability and warranty disclaimer page:


Page 2 is your certification that you're eligible to purchase the firearm (basically the background check info) that needs to be notarized.


Page 3 is the actual order and payment form.


And Page 4 is the checklist.


All forms need to be completed in full or your order will be invalid and paperwork destroyed.

See what I mean? Pain in the arse, but worth it if you're wanting a collectable piece of American History!
