I run a forklift service department and on some of our newer models the factory put the DEF fill directly above the fuel fill... Idiots. Same customer has twice now filled the DEF tank with diesel and ran the unit. $8-9k a pop. Kenworth was smart. Our Peterbilts are the same way. Opposite sides. Otherwise we would probably be short on drivers.

My Volkswagen has a slick system. The little flapper over the fuel fill tube will only open if the nozzle is as big as the opening. There's two little tabs about 90 degrees apart that have to both be depressed for the flap to open. Since gas nozzles and urea nozzles are smaller, they won't engage the tabs and the flap won't open unless the human is hell bent on making it happen. Diesel fill nozzles are bigger and go right in. Not sure why that isn't implemented on all diesels, especially ones which have the urea fill right next to the fuel fill.
