Why is it so hard to find nice shoes with removable insoles/inserts today?

I have to wear orthotic inserts as I have really flat feet. 10-15 years ago I could buy decent shoe brands such as J&M or Bostonion (before those guys went down) that allowed you to remove the inserts and put in what worked for you. Now they are all attached. AE makes a line of removable insoles and they fit great but they are cheaper leather and soles.
Americans spend about a billion dollars a year on orthotics. Why do the nicer shoe manufacturers ignore them or assume they want cheap crap?

Cole Haan went the orthotic route a few years ago but I think they ditched it.
