What happens if the slope is zero? Can you skim with extra concrete years after to increase slope?

No. There most likely would not have enough depth to effective build up the "top coat." Meaning it would flake off. A coating could be an option but it would be epoxy based and not cementious.

Yes, there is usually a couple inch step down from the slab. You should be able to build it up on the high side and slope it. I would read the manufacturer recommended thickness for both maximum and minimum. I would use an epoxy product.
