Smith is in the Top 3 surnames in 45/50 states - can you name the other 5?

Came across a link to an Ancestry.com blog about the top 3 surnames in each state. Smith is #1 in 40 out of the 50 states, and in the top 3 in 45 out of 50. Educated guesses can probably get 2 of the 5 states where Smith was 4th or worst, but the other 3 are pretty random. Anyone have a clue what they are?

Boudreaux and Thibodeaux should be pretty high up for Louisiana.

Louisiana is one of the 10 states where Smith isn't #1. In LA, it's:

#1 Williams
#2 Smith
#3 Johnson
