Revision knee replacement surgery

Good day. I need to have a total knee replacement revision surgery. I was referred to a orthopedic surgeon at Baton Rouge General Orthopedic who apparently is a great surgeon on revisions. Watched his video about pain management after surgery and is not too keen on using opiods except for acute pain post-op. I've had 3 knee replacements already. Can someone suggest a great orthopedic surgeon that does believe in using opioid for longer than 3 day post op for a T.K.R? I'm not a drug seeker. I just prefer not to hurt too bad.

A script for 3 days worth after a knee replacement? Are you sure you watched the right video?

If it’s my fourth knee surgery (original Knee replacement with two subsequent revisions—assuming it’s the same knee), I would be worried more about the quality of the surgeon as opposed to the days of pain meds.

Or just go to a SNF for a week post surgery. They won’t worry about your opioid usage until discharge.

A video on what he does after primaries may not reflect what he does after revisions. Also how much/long you will need the medication may be dependent on how extensive of a revision you need. Less opioids is the new norm so regardless of where u go you likely won’t get a long duration

The implant that was put in the 3rd time is too big and a tendon is catching on the implant on the lateral side which has now stretched the tendon. It was also placed to far laterally. So both the top and bottom implant has to come out. They were cemented. I have a feeling it will not be a pleasant experience.

Lots of knee replacements are discharged the same day as surgery these days. Typically the 65 yo lady with bad OA who still plays tennis 3x/week.

Any comorbidities? (DM/HTN/CHF)

C.O.P.D., that's it. And I'm a little overweight. The last replacement, 4 years ago, before the media started beating the drum on the opoid epidemic, I was hospitalized 3 days, hydromorphone pump for a day and a half, plus oxycodone every 6 hour. At discharge Norco 10 q 6hrs for a week.

I think a week of norco/Percocet (28 tabs) is what you can expect these days. Something to discuss in the preop appointment.

You walking ok now, or using a cane/walker?

If it’s not infected you should get exparel. That will help with the pain but to completely avoid opioids will be tough

ETA: I misread. You want opioids

Not not walking well at all. I also have a serious bulging disc at L-5/S-1 that I believe has been caused by the knee issue. I've lost 4 cm of muscle mass in the affected leg vs normal leg. I've been through this before and I think alot is the anxiety of going through the pain since the government is wanting doctors to reduce the use of opiods.
