Yet its happened numerous times. It has nothing to do with location, it has more to do with reacting under stress.

I don't know if it really matters- I believe the relevant standard is based on what a reasonable officer believed at the time of the shooting (Graham v. Connor)- not afterward.
With that in mind, how did the officers know he was high? Like I said earlier the video I saw didn't show a lot of interaction between the driver and the police.

I suppose, but I'd like to see the setups for the officers who have had "accidents".

Him being on drugs would explain a lot. He was acting really stupid if he was basically ignoring everything the police were saying to him. Also why was the car broken down in the middle of the road? Wouldn't be try to pull it to the side when he realized he was having trouble? It sucks but he really didn't help himself out at all. The thing that gets me though is how they didn't even help him out at all and just let him bleed without giving him medical attention

A decent article on the subject, though it's a bit naive IMO.
Cops are trained that the problem isn't the other guy, it's their own complacency. Being complacent gets you killed. It's drilled and drilled into them. In some ways I think it creates confusion in dealing with less than lethal situations. I think many police training programs neglect the less than lethal approach and officers aren't always accustomed to using it. They go with their instincts which can be deadly.
