What that does is really affect people that are "truthful" in their responses. I am very conservative when I go in with chest pains (pre-heart attack). I got a doc in ER once that didn't know me and didn't believe I was at a true 8. By the time they did my stent I had been at 10 for over a hour. I would have killed myself if I had the means. When it was all said and done the heart doc told me the left side of my heart was collapsing, my blood pressure was unbelievable, and it was really "close". Asked him to put on my records that I don't exaggerate pain and tell his ER doc to go frick himself.

I hate taking opioids. You give me anything with codeine or hydrocodone in it and you would think that I just snorted an eight ball of coke at one time. I don't have any pain, but I sure as frick cannot sit still and it feels like my body temp goes up about 10 degrees.

Luckily I have a high tolerance for pain and very rarely have ever had to take them. Had all 4 wisdom cut out back when I was in high school and never took a pain pill. After about 4 days my mom took the bottle from me, though, because she knew I would sell them. Had a really bad car wreck as well that I thought I broke my neck. They had to hold me up at the emergency room to take x-rays. Was laid up in bed for 4 weeks after and never took a pain pill.

I laugh inside at the patients who come in saying they are in a 10/10 stage and everything you do makes them scream in pain. Yet while your typing they are content to be on their phone or talk to their friend who came in with them. I always act scared for them and send them directly to the ER. They never return saying they are in pain.

Different people have different pain thresholds.

I generally have a pretty high threshold, which has caused me problems over the years. (for example, once got blood poisoning because I let go an infection caused by a piece of glass stuck in my foot - it hurt a lot but I was inclined to 'tough it out' until my foot swole up, turned a nasty color, red streaks were running up my leg, and I couldn't walk on it any more. I got a few stories about lacerations, broken bones and kidney stones, and operations, too... )

Other folks have lower thresholds so a bang on the shin might be excruciating...

Bottom line is the pain index is only a point of reference and there are no absolutes in its application.

Regarding pain killers of any kind -especially opioids or opioid-like substances- I don't want em and suggest that no one should unless there is absolutely no alternative (that said, I have had to have morphine in a couple of hospital/emergency room situations).
