OT Doctors

Calling the doctors of the OT board, I could use some help. My dad is a bad diabetic and for about the past 4-5 days his blood sugar has consistently been between around 200-400 and it will not come down. He's been giving himself insulin at the normal times but it hasn't changed. On Wednesday we took him to the ER and he was dehydrated so they gave him some fluids and sent him home. He's been drinking a shite ton of water since then. But his blood sugar WILL NOT go down. He checked it about 20 minutes ago and it was 410. Any ideas on what could be going on?

med school student here and also 20 year type 1 diabetic

has he had any illnesses recently?

what insulin is he taking and is taking any other medicines?

Are you sure the insulin is good? It seems like when my mom had some that had expired or that had been open for too long, it would become less effective at controlling her blood sugar.

go back to the er.

That happened to a close family member of mine and they died from a stroke. That very day.

Not saying all out panic but, I'll never take "I promise i'll go to the doctor in the morning" as an answer ever again.

is he nauseated?

this starting to sound like he is right at the cusp of DKA

what is his age?

I'm shocked they didn't keep him at least over night for IV fluids

No illnesses. And he takes Humalog and Lantice(sp?)

is he using a sliding scale for the humalog?

what is his diet like?

have you tested his urine for the presence of ketones?

If he's drinking a lot, you know his sugars are up there. I'd be getting him back in the Hospital..

No nausea but he has been weak, and he's 71

with his latest blood sugar, how long ago had he ate?

what is his medical history? Any other medical problems?

He ate at 6:45, and it was around 350 then before he went to bed it was 410 and had given himself insulin right before then

the humalog needs about 15-20 mins to act and get a BG down from that high of a level

still, to be that high after, what, 4 hours after a meal is not normal.

IT could be that his lantus dosage needs to upped or that he did not give himself enough humalog with the meal

but to cut to the point, he needs to go to the ER now. At his age, this is not something you chance. If he is headed towards acidosis, he certainly does not need to be going to sleep
