An Amtrak train derailed in Philly last year due to human error by the train engineer. So yes, these kinds of things happen "accidentally."

Sure trains derail due to human error but to plow full steam ahead into a station seems highly unlikely

Maybe the driver had a heart attack and was unconcious

Yeah, Hoboken is an end station or whatever you call it. I believe trains start or stop there instead of passing through. Seacucus is usually the first stop for trains coming from NYC.
