Good god.

If this happened at work:
- all work stopped
- immediate safety stand down with all craft
- prompt investigation of the worker and supervisor behaviors
- document coaching and / or discipline
- fleet memo of the incident including actions taken to correct the behavior
- meeting with leadership to allow craft to return to work
- brief out at the morning meeting on the incident
- increased observations focusing on safety and human performance behaviors for min 4 weeks
- root cause to evaluate the ladder to see if their was some failure of procedures that need to be corrected.
Etc. etc. etc.
