Somewhere that's a person that needs lungs that doesn't vape and isn't obese that isn't going to get lungs now and this guy is just going to burn his new ones out with his vape habit.

I've met fat people who exercise regularly and eat clean
I'm gonna go ahead and say you're an idiot.
If someone is eating clean (ie. No/ little trans fats, less calories in than calories out) and exercising for 30-45 minutes 4-5 days a week.
Just because you have a salad for lunch and jog 20 minutes twice a week doesn't mean you eat clean and exercise regularly.
Maintaining non-fatassness is as simple as calories in < calories out. It's not magic.

That’s him after 3 months immobilized in hospitals. How big was he when it all started?

You think he got bigger eating hospital food?

No, I assume he lost about 100lbs.

I would have guessed he lost 75ish.

Most lung transplant patients don't live that long. Eventually the body rejects the transplant and it becomes a grueling battle of constant hospitalization and extreme meds to ward off the immune system's rejection process until eventually the cure is worse than the disease.
I think 50 percent of them are dead less than three years after transplant and 75 percent are dead after 5 years. It's just buying time.
