The fifth kid would have made him the middle. Malcolm was the 3rd of 4 boys originally: Francis, Reese, Malcom and Dewey.
Now, he was in the middle of the primary featured 3, as Francis was not living at home.

One of my favorite episodes is when she's pregnant and the dad does everything he can to try and get her fatter, because he was turned on by plump preggos. Dude was lacing her milk with sugar, etc. Hilarious.
