It was the summer after I turned 15, and I had my learners permit. My grandmother in west Alabama had a stroke, and was in the hospital in Meridian. We were staying with family in Alabama and driving the half hour or so to the hospital every day. Everyone took turns driving over, and my parents and uncle always let me drive their car to get practice in since it was a short trip down a small highway.
My mom had a little Nissan 240sx, and that particular day I was driving us to the hospital that morning. We were on this little 2-lane highway, and I was coming down a hill and around a slight curve. I saw a big dump truck pull up to an intersection at the top of the hill and stop. I was going about 60 or so, and climbed the hill pretty quickly. I guess he didn't see the little car, since I was approaching from his right and he was pretty high up in the truck. He starts to pull out, and my only option was to get off the road. No time to slam on the brakes or we would have impacted the cab or ended up under the back. I laid on the horn and swerved to the right, and glanced to my left as I passed in front of him and saw the big MACK letters on the grill about 2 feet from us. I think I scared my Mom half to death. She was reading something in the passenger seat and didn't realize what happened until it was over. 45 minutes later I was still shaking from it.
That and the time I nearly drowned as a child. Fell into the deep end of a pool and couldn't swim yet. A counselor dove in after me, drug me out of the pool, and performed CPR. I had to spend the night in a hospital for observation.

The odd thing about when I came close to being gone for good is it never occurred to me at the time that I was dying. I lost consciousness around 6:00 PM that evening and did not flat line for another three hours.
However, a week before that night, the day of my first open-hart surgery, I was about 100% certain that once they put me under for the surgery, I was never going to wake up. The surgeon had already warned me and my wife there was a pretty high risk that I'd not make it*. I put a brave face on it that morning but when I was telling my wife and kids bye that morning, inside I was certain I'd never see them again. The odd thing I remember about that morning was I was very sad that I'd never see them again and miss so much of my kids growing up. But I was not that scared, instead I had peace and acceptance about it that was very calming.
* I did not know it at the time, but my cardiologist figured I had about a 10% chance of surviving.
