It's most probably settlement. The catch basin/drain does not settle at the same rate as the pavers around it because it's buried deeper and most probably on stiffer ground.

I've seen this a lot in Florida, in freshly built subdivisions.
Or, the man holes are 6 inches above the asphalt

All depends how old said area is.
Looks like the contractor fricked up the grades at first glance....however, the failure could go to soils consolidating underneath the pavement/pavers. Did they compact it to spec? Did the geotech investigations get the soil types wrong?
This assumes the bedding of the box was done properly and said surrounding receded.
That buckled paver may provide a valuable clue.
