Call the city or parish permit official. Have your neighborhood and lot number info along with your address. Should be nothing to them giving you a very quick answer.

Almost anytime you put a driveway in and tie into the road you are pouring over lines.
If it is the line going to your house, put a proper sized pull conduit through at the proper depth. If there is ever a problem you can pull a new line.

Yes you can. Every slab construction I've ever seen does. Just prep it right and it's no problem
