Of course we dont get to see the terms of the contract but there has to be some sort of breach of contract remedy to be able to identify when this injury occurred. I suspect to keep all the laundry clean the only remedy for the Saints is arbitration with the NFL selecting the arbitrator. File for arbitration, put Carrs salary in trust and do this:

That's what will happen if he is somehow cleared to play. With his injury guarantee, there is no reason to risk playing him.
Keep his arse on the sideline with nothing but bad PR and annoy him until he's willing to take a pay cut in exchange for a trade or release.
You can probably get a sponsor or maybe a few rotating sponsors to pay for the Derek Carr Update during press conferences. Just to low-key aggravate him by reviewing what he's doing presently.
"Now, we have the Derek Carr Update sponsored by Paretti Mazda. Derek Carr is still doing nothing. His practice reps are 0 for 0 for 0 yards. He did have one stupid car review lately though. Way to go Derek; enjoy your time off."

With the cap ramifications and Carr’s attitude, make him the 3rd string emergency inactive QB each week leading up to the trade deadline.
If a contending QB goes down with an injury, Carr may waive his no trade clause during the season.
Otherwise, just embarrass him each week with his inactive status—especially if he continues to fake an injury without an actual surgery.
