What's the difference between the teams that play in both Vegas and other places like Sacramento or Utah? And how do the foreign national teams come into play?

I'd help you out, but I don't know much about it myself.

Teams can choose to play in whatever league they want. Vegas is the big one, in the past it used to be Orlando and Vegas with WC teams gravitating to Vegas and EC gravitating to Orlando, but not exclusively at all.
I honestly didn't even know the Utah and Sac ones existed until this year.
Its basically just a practice and tryout league. Its gotten huge in recent years but it really isn't much more than that.
As for the international teams they usually invite 1 or 2 just to compete and grow the game internationally.
