Die Hard 4, the villian erased McClain's 401k and saw his pension and stuff like that, but didn't look at his records. If he had, he would have seen what had happened in the first 3 movies and known what he was dealing with. That bothered me

I love the movie Unforgiven, but there's one scene that really bothers me. After the Schofield Kid shoots the guy using the outhouse, he and Will Munny jump on their horses to make their getaway. About 5 or 6 guys start shooting multiple shots at them, CLEAR shots, and not one bullet hits them or their horses.

Bad guys really are terrible shots with all form of weaponry.
What about Battle Star Galactica? Every time the Cylons attack the Galactica, they send out hundreds of cylon fighters and the humans send out a few dozen fighters and the cylons almost never kill a single fighter.
