There's two different types of policies. Total disability and loss of value.
Total disability is difficult to recover because you have to show a single injury that caused disability. These guys have so many injuries that Lloyds can point to and say that's the reason they weren't drafted/signed. They can also say that the injuries weren't the cause of the guy not being drafted/signed. Basically, it's like most insurance in that they will try to nickel and dime their way out of it and make you get an attorney. But ultimately, they always pay something.
Loss of value is a lot more straightforward, but they're only available to the top tier guys. They basically look at where you're projected to be drafted and insure against any drop in that contract money that's caused by an injury. Much easier to recover but again they have some wiggle room to argue that a drop in the draft was not caused by an injury.
Also, the NCAA allowed these policies to be funded through loans and the schools a long time ago to encourage the athletes to get them.

Nothing new . Marcus Lattimore Had a 1.7 million policy in 2014. Granted top guys would take a loan to do it
