Garza, Hilliard, Hill, Storz, Dugas, Labas (in 2018), CJ Willis, Hughes, Hess, Watson, Reid and Marceaux (in 2019) come immediately to my mind without doing any research.
All of those players either aggravated their injuries when they returned to play, came back for a game or two and then had to sit again or when they were cleared to play by the trainer after an injury they couldn't perform anywhere near 100%. Dugas couldn't even grip the bat with his left hand when he returned to the lineup after surgery for a torn ligament in his thumb in 2019.

I'm not one to criticize players or baseball staff. I believe my posting history has shown that. But our baseball trainer function needs improvement when it comes to managing player injuries. If Mainieri is at fault by rushing players back into the lineup against the trainer's recommendation (which I doubt) then he needs to do better.
A hamstring specifically is fickle. We've discussed this already. It can feel 100%. You can run and cut and nothing feel wrong. But then in a game, the adrenaline and moves aren't the same as in practice. It's just too difficult to simulate. That being said, we have someone here who seems to have known that his injury wasn't 100% before the game. If he knew, I would hope PM knew.
