Have just a couple small spots of very stubborn black mold/mildew. Does this mean that water is seeping through cracks in the grout and there is a larger problem underneath? Or is there not enough information to know this definitively? Not sure if I need to get someone out to rip this up and redo it or what.

No, it doesn't necessarily mean there's a leak. It can mean that your shower doesn't dry quickly after use, and the mildew persists in the spots where surface water collects/lingers--do you have a vent fan? Do you squegee out excess water after showering? Have you tried scraping out the grout in those spots to see if there is subsurface moisture?
