As a physician I say without a doubt that Type II DM IS a choice - if these fatasses lost those extra 100 pounds this disease (along with their cholesterol problems) would go away - but this is hard and requires work and dedication which most folks don't have
And the ADA is vested in having these people in victim mode so that they have someone to campaign for

I look at type II DM this way - yes there is a genetic component, that just means you have an "inefficient" pancreas that can't handle as much sugar as others. But people make the lifestyle choice for high carb/processed crap that goes over what their pancreas can handle. Stop the sugar overload and many of these people can stop all of their medicines.
Why doesn't it happen all the time? Because it is hard work and most americans don't have the intestinal fortitude to do it. It is a lot easier to take pills or insulin than hit the gym.
Yesterday I had a conversation with a patient where he was maxed out on one oral medication and I told him we would have to add another or he could really improve his diet and exercise. He looked at me thoughtfully and said "Nope, I like my junk food too much. Give me another pill."
Some days I feel like I'm going to get a worker's comp injury from shaking my head in derision so much

Seven years ago I was pre diabetic while weighing 400+ and it took a no nonsense doctor to tell me I was going to end up just like my father who died at 44 because he simply wouldn't take care of himself.
