I heard a story about a guy who used to come to our Applebees, and would lay 5 $1 bills on the table, and tell the waiter/waitress that every time he had to wait for a refill or anything else, he would remove one. That was their tip. 
I had one guy tip me bad and wrote a note on the bill saying it was because I kept saying "how are ya'll doing". Said it wasn't profession. Applebees.
I'm not country at all, but living down here for so long.......kind of gets imbedded into your vocabulary.

if a guy did that with the dollar bills, I would immediately leave, tell the manager, other servers and just let him sit there or have him ejected.

Well, I'm guessing that Fox uses it as a general term for bad tipping people, but that was our code for black customers. "Canadians at table #31". Like I said, a black waiter told me about it. Said he admitted they were crap tippers. He didn't even want to wait on them.

i would say 15% is fair, but since you've stiffed, it sounds like several, servers you should tip 20%
for example your bill is $55.00...double the first 2 digits...and put a decimal in the middle
