For those that consistently use them, what is the best method for keeping the bag fully submerged?

I just use the biggest utensil I can find. A big pair of tongs or a spider/strainer.

I haven't had any issue with that. I sometimes use a clip to keep the bag away from the appliance. I have also blocked it with a filled beer glass.

I use a butter knife inside the bag to weigh it down if the food is more prone to float.
