Can you get that done at a doctor's office or do you have to go to a specific facility?

If you've read the book, they specifically tell you that snacking the first 5-7 days is fine as you are severely changing your intake. Now, they do tell you not to do chips and bars, but I found that week 1 those were a must.

Thats a huge part of my wife and I's success at it. I would grill 5-6 chicken breasts on a Sunday for "In Case" meals. Or she'd do a big crock pot of compliant pulled pork. To have that food just to eat and not have to worry about it makes it so much easier.

Only certain clinics would offer that.

I spoke with an Alletess rep and she said that a doctor can send them an order, and they can mail you a card which you fill up 4-6 circles of blood after doing a fingerstick.
