How old are you?
Have you had any other moments that the shoulder gave you those same symptoms? How has it felt since that one instance? Have you had any actual dislocations or just the pain? Are you naturally very flexible?
MRI would be interesting to show location of tear.
To answer your question, surgery is always an answer, but may not be completely necessary first thing. You can always try to be patient and rehab it, but you probably need some guidance to really work your way through it. If that fails, surgery can always still be a later option without any detriments waiting.
Also depends on what your goals are, though it sounds like you’re lifting pretty heavy. You certainly could still lift (heavy too), but you would have to be a lot smarter about your programming. Also depends on if it’s your dominant arm or non dominant. A lot of things go into it, but I will say if you’re going to be doing a ton of rehab anyways, might as well try and be conservative. Once you mess with your natural anatomy, you can’t go back

27 never had a serious injury before. Just strained muscles and an impingement on the opposite side that I was able to self rehab (thanks YouTube) didn’t take long to get back to 100%
It is on my dominant side and pain has taken some major steps over this past week. No more wincing from pain when reaching behind my back. Was able to do some girl push-ups slow and controlled to test stability. Wasn’t painless but stability and strength felt good
No previous dislocations and shoulder flexibility has never been an issue
I do really enjoy lifting heavy and would like to get back to pre injury strength numbers
