What are the steps for FDA approval?
-Preclinical testing
---This is the lab and animal testing phase. Average testing time: 3.5 years
-Investigational New Drug Application (IND)
--The pharmaceutical company files an IND with the FDA to begin testing the drug in people. The IND becomes effective if the FDA does not disapprove it within 30 days. The IND must include the following information: the results of previous experiments; how, where and by whom the new studies will be conducted; the chemical structure of the compound; how it is thought to work in the body; any toxic effects found in the animal studies; and how the compound is manufactured. The IND must also be reviewed and approved by the Institutional Review Board where the studies will be conducted.
-Phase I Clinical Trials
--This is the first round of human volunteers, usually 20-80 healthy individuals. Average time for Phase I: 1 year
-Phase II Clinical Trials
--This phase expands to 100-300 people who have the disease for which the drug is intended. This is a controlled stage used to test the drug's effectiveness and determining min/max dosage.
-Phase III Clinical Trials
--Usually involving 1k-3k people this test to further test efficacy as well as negative effects (used in conjunction with negatives found in Phase II). This is where you will find double-blind studies with placebos. Average Phase III time: 3 years
-New Drug Application (NDA)
--The manufacturer submits all of their data (including how the drug is made and it's properties) to the FDA. These things are massive, generally consisting of 100k+ pages. The average NDA time (as approved by the FDA in 1992) is 30 months (so almost 2.5 years).
-Phase IV Studies
--After-market studies once a drug as been approved by the FDA. Patients can still report symptoms and such in this phase.
Even with all of that we still get drugs which end up having enough negative and/or long-term issues which testing did not catch.
With all that said there is a fast-tracking process, that process is 60 days.

Thalidomide Note: the FDA never approved Thalidomide, it was in testing when the problems became known. Over 2.5M tablets had already been given out to over 1,000 physicians in the country and causing at least 20,000 birth deformities. Had a lone FDA reviewer ( Frances Oldham Kelsey) not stood up to pressure from the supporting pharmaceutical company, this drug likely would have made it into far wider use.
