Ours is inside the house towards the center of the house, and the elevator shaft was framed out during construction. We had to make a 4 to 6-inch pit in the slab so the car would be level with the floor on the first floor. Ours is from inclinator and is similar to this LINK but with the old 80s style cab.
I would guess you could have an addition or bump out added for the elevator. Going to raise the cost though.
