In none of his videos does O'keefe ever show the questioner on the video. The closet he ever came is when he had his "informer" asking the questions. But the informer was behind a screen and the voice was disguised and garbled.
Why is it that O'keefe's questioners are always offscreen? You can never verify that they are actually asking the questions on the audio.

Secondly, this makes sense for undercover recordings. Publishing their faces ruins the entire undercover part of the job.
