In a nut shell, you remove all of the emissions related components on the truck and reprogram it to run normally without it.
Of course this is illegal for any on-road vehicle in the US. It will greatly improve reliability via removing lots of parts and improve fuel mileage via eliminating regens, and possibly pre/post injection depending on the programming.
Again, illegal for on-road vehicles. Not difficult to do the hard part work, more difficult to find someone to program it.
