You’re most likely leaning back. Try punching shots on the range until you hit it square. Feel like 90% of your weight is on the lead foot.
Remedy #2- Find an old (1970s) set of blades and hit them on the range. You’ll learn to strike it or else

I make a line with two tees 18 inches apart.

In my prime in the '70s I owned a set of Staff persimmon woods & blade irons and later bought a set of Ben Hogan persimmon woods & blade irons.

Fairly confident this is my key issue. I swing flush if I take a practice swing but I think I lean back when starting downswing.
