The release will be for future medicals. There is no other reason for the claims adjuster to just randomly give you 1k out of the kindness of their heart. You do have options though. You can accept, negotiate a higher amount, or not accept just in case you do have medical bills related to the accident at a later date.

I would wait for the foreseeable time. This won't be the last time they talk to you about signing a release. At that time you'll have more information with the health of your family to make a informed decision.
