Great Q! I don't KNOW the answer, but here's what I think I know:
- the official name was in Arabic numerals, so "Apollo 11"
- any use of Roman numerals is "stylistic" but more or less correct
- all Apollo missions "official name" was in Arabic numerals
- all Gemini missions "official name" was in Roman numerals
- the appearance on the PATCH does NOT necessarily indicate an official name - see below
But feel free to correct me if I am wrong.

I can't correct you because I haven't closely studied the Apollo program. It is interesting that Apollo 11 seems alone in using Arabic numerals.
