And that is why the price should be the same everywhere. It is a product and costs XXX to produce, it should not be sold to a poor country for less because those people are poor.
Gasoline does not cost less in the west end of town.

The "West End of Town" is still in the same place as the high end income of town. Try the same size town in Mexico, Guatamala, Rwanda, Congo, Viet Name, India, Ukraine, even UK, Canada, NZ.
If I've made 100,000,000 pills and I need to sell them I start in the USA at the highest price possible, but if I have 1 or 1000,000 left I sell to the next highest at the most I can get out of them, all the way down the list till I run out of pills...then I make more and repeat. That's how business works.
