you been playing anthem?
The issue just seems to have come out of nowhere. I wonder if this is common with PS4's after a few years of heavy use?

Describe heavy use. I run my everyday for several hours at a time. I've not had this issue.
Did you get it fixed?

I've had this issue after a power failure improperly turned the console off while in use. Just had to do a hard boot and it rebuilt it's database on startup.
No idea what to do if yours isn't doing that.
Might be worth it to make sure the hard drive is still seated correctly. Could also be an old hard drive failing. If that's the case Sony made them really easy to replace.
Good luck.

I have a normal PS4 that I bought in 2014 and all I’ve done to it is change out the hard drive.
Only problem I’ve encountered is the ghost beeping and constant disk ejection and that was solved for the most part by removing the rubber bumper piece you would push to power/eject a disc.
You may have to start in safe mode and do an OS rebuild or whatever you call it.
