Get an MRI

What will this tell me, whether it is structural or muscular?

It will tell you what is going on with your spine and give the PT, Ortho, whatever a good idea of how to go about addressing the problem.

A friend of mine recently went through this. PT didn't work. Chiro helped. Got an MRI and found out one leg slightly shorter than the other and it was causing the back pain. Had to get orthotics. Orthotics + Chiro and he has a lot less pain now.

You're situation could be different but sounds very similar.
