I haven't watched it in a very long time, possibly going to do a rewatch soon, but I don't think Tony died in that diner. I think the sense of dread you feel watching it, looking at everyone in the diner and waiting for them to kill Tony is the dead he will feel. His crew is gone, someone in the family might have flipped and his world is now just waiting.
