Have you used the NHS as an English citizen?
Also, the NHS is purported to be the world's 5th largest employer.
Yeah, a country of that size employs enough people to be the world's 5th largest employer.

3. Walmart
4. McDonald's (must include all the franchises obviously)
5. NHS

In the last several years, satisfaction has been between 60-70%. That's pretty good.
