I know I'm taking this snippet and commenting on something not entirely related to the opioid problem, but this fact above blows my mind.
Why do we have to fill out a medical history form every time we visit a new doctor? Why can't this information be shared? I'm assuming HIPPA, privacy concerns, etc?

100% due to privacy concerns and HIPAA. There are regional organizations that many healthcare systems take part in, but nothing as far as a national database is concerned. Also, healthcare systems that share the same EMR can pretty easily share info, but you have to know which hospital to request info from.
