correct, it's not hard at all
it's a matter of technique
I can do it at home, and I'd expect a place charging 12 bucks for a "gourmet" burger could do it as well or better

dry = flavorless
medium rare to medium is the sweet spot

ill say i went to shank - new butcher over on st claude in nola.

fresh ground patty, pimento cheese, big ol texas toast slices..... was cooked medium (pretty spot on to the temp).... was excellent.
